About the role
Managing an assigned member caseload, the full-time New York Licensed Care Coordinator will perform care management services, assist members in navigating their healthcare system, and support various teams to ensure excellent customer service remotely.
Key responsibilities:
Receive and document incoming calls from members regarding benefit questions, care coordination, and screening assessments
Collaborate with the interdisciplinary care team to communicate member needs and monitor screenings for quality improvement
Conduct outreach calls to confirm that services are in place for members requiring short-term care coordination
Required qualifications:
Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training or experience
High school diploma or GED
Experience in a fast-paced environment managing multiple priorities
Effective verbal and written communication skills with good grammar and spelling
Familiarity with medical terminology and healthcare environments is preferred
