About the role
A mission-driven organization working at the intersection of food, healthcare, and technology is hiring Care Navigators to support New York's Medicaid Waiver program. In this role, you'll be the first point of contact for Medicaid members — screening for eligibility, assessing Health-Related Social Needs (HRSN), and connecting members to the services and community partners that can help them live healthier lives. The Details Fully remote Monday–Friday, 9a-6p + 2 Saturdays per month$25/hour Bilingual required — Spanish or Mandarin*must reside within 60 miles of NYCWhat You'll DoManage inbound and outbound member communications by phone and email using Five 9 and partner platforms Screen potential members, assess eligibility, and navigate them to the right service providers based on their needs Track and follow up on referrals to ensure services are delivered Maintain accurate documentation across program and internal systems Support members with program questions Comply with Medicaid, HIPAA, and NYS DOH requirements
What We're Looking For:
Associate degree required 3+ years in customer service, call centers, social services, care navigation, or a related field Experience in a virtual contact center (Five 9 preferred)Strong communication and member-engagement skills Comfort managing high-volume interactions in a compliance-driven environment
Nice to Have:
Experience with Medicaid, Social Care Networks (SCNs), or HRSN programs Background in healthcare, care coordination, patient support, or community-based services
