About the role
CVS Health is hiring a Case Management Coordinator for Northeast Ohio to coordinate care for children and families under OhioRISE. You will travel to member homes and sites up to half the time, collaborating with care teams to address needs and optimize outcomes.
Requires 2+ years in behavioral health or related fields, plus proficiency with MS Office and Share Point. Bachelor’s degree in behavioral health or related area is preferred, with a strong focus on regional travel and after-hours
