About the Opportunity:
Provides a dynamic link between clinical practice and service development to support new models for delivering patient care. Understands the facilities unique needs and provides a tailored program that encompasses the educational, regulatory and cost-containment concerns of the facility while assuring appropriate medication therapy for each resident. Relies on experience and professional judgment to plan and accomplish assigned tasks and goals.

What You'll Do:
Coordinate pharmaceutical services within a long-term care facility.
Collaborate with facility leadership and healthcare professionals to improve resident care.
Develop and evaluate pharmacy procedures, policies, and best practices.
Review residents' medications and identify pharmacy-related concerns or opportunities for improvement.
Provide recommendations related to medication management, clinical care, cost savings, and formulary compliance.
Prepare quarterly reports on drug utilization, clinical trends, and pharmacy services.
Provide medication-related education to residents and staff.
Participate in Quality Assurance Committee meetings and provide required reports.
Ensure compliance with federal and state regulations regarding medication ordering, storage, labeling, handling, and administration.
Review medication storage areas, emergency drug supplies, and medication labeling for accuracy and compliance.
Assist with regulatory surveys, audits, and pharmacy-related inquiries.
Report dispensing incidents and maintain required documentation.
Maintain professional relationships with physicians and other healthcare professionals.
Maintain patient confidentiality and comply with all pharmacy regulations.
Complete required training and perform other duties as assigned.
Job RequirementsWhat You'll BringMinimum: BS or Pharm D degree in Pharmacy
Posses and maintain a valid State License and current registration to practice Pharmacy
Ability to stand/walk throughout majority of shift
