Job Description
Speech-Language Pathologist - Home Health:
ScheduleSelf-scheduled visits, with a requirement of 6-8 home skilled visits per day. Availability for 1-2 weekends per contract.
Assignment Details:
Work requires travel across multiple territories. Contract lengths vary based on the needs of the office.
Requirements:
- 1 year of home health experience required
- Valid driver's license and personal vehicle for home visits
- Certification in BLS and CCC-SLPState Licensure requiredOASIS training preferredCOVID vaccination or exemption form on file
- Flu shot or declination documented, with mandatory mask wearing while at work
Responsibilities:
- Conduct evaluations using various screening tools such as BIMS, depression screening, and oral peripheral exams
- Develop and implement dysphasia management and aural rehabilitation plans
- Educate patients and families regarding therapy plans and patient safety
- Maintain documentation and billing in compliance with regulations
- Provide in-service training as required
- Implement functional maintenance programs for patients
