About the role

Canopy Children's Solutions is seeking a Wraparound Facilitator to engage families, develop a strength-based plan, and assemble a Wraparound Team to support children with complex mental health needs. You will coordinate with family, youth, and community partners to create a holistic single care plan and conduct regular team meetings, ensuring crisis plans and documentation are completed promptly. A Bachelor’s in Social Work or related field is required.
