HealthPartners is seeking a Pharmacist to join our MTM team in ambulatory care. The role focuses on improving medication outcomes through collaboration with a multidisciplinary team, ensuring safe drug therapy, and enhancing patient quality of life. The schedule includes weekdays with a mix of in-person clinic locations in St. Paul and St. Louis Park and a blend of in-person and remote work days. Minnesota and Wisconsin licensure is required.
