About the role

Cardiovascular Associates of America is seeking a Chronic Care Management Health Coach to support patients with chronic conditions in a remote, full-time capacity. You will develop coaching relationships, monitor care plans, and liaise with the cardiology care team to improve outcomes. The role requires RN/MA/CNA credentials, remote CCM experience, and familiarity with EMRs (Athena, NexGen, GEMMS, e-clinical works). Salary is $20–$24/hour with benefits and remote work options.
