Position Overview: We are seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, and help residents improve mobility, strength, independence, and overall quality of life. This position works closely with patients, families, nursing staff, rehabilitation professionals, and other members of the interdisciplinary care team.
Responsibilities: Perform comprehensive physical therapy evaluations based on physician orders, payer authorization, and facility guidelines. Develop and implement individualized treatment plans based on each patient's diagnosis, needs, and functional goals. Provide skilled physical therapy services designed to maximize mobility, function, safety, and independence. Monitor patient progress and modify treatment plans as appropriate. Complete evaluations, daily progress notes, summaries, monthly reports, and other required clinical documentation. Enter patient information accurately into EMR and rehabilitation systems. Identify and screen residents who may benefit from physical therapy services. Educate patients, families, caregivers, staff, and other healthcare professionals regarding treatment plans and functional outcomes.
Qualifications
Physical Requirements: This position requires the ability to move throughout the workday and assist residents with mobility and therapeutic activities. Duties may involve extended periods of standing, walking, or sitting, as well as bending, squatting, pushing, pulling, and lifting or assisting with patients as appropriate.
Required Skills
Required: Graduate of an approved/accredited Physical Therapy program. Current, active, and unrestricted Physical Therapist license in the state of practice. CPR certification as required. Strong communication, documentation, organizational, and customer-service skills. Ability to work effectively with patients, families, caregivers, and interdisciplinary healthcare teams.
Preferred Skills
Preferred: Previous experience in long-term care or skilled nursing. Knowledge of PDPM.Knowledge of Medicare and other payer sources. Experience with NetHealth and PointClickCare (PCC).Knowledge of group and sensory therapy. Pay range$60-$75 an hour Equal Opportunity Statement
