About the role

This is a full-time, on-site position primarily based at a hospital location, requiring flexibility to travel between multiple sites and participate in weekend, holiday, and on-call rotations. The role involves acting as a liaison among patients, physicians, hospital staff, and home health agencies to coordinate and plan home care services. Responsibilities include contributing to discharge planning, documenting care coordination activities, and educating staff and the community about home care and specialty programs. The position requires multitasking, prioritization skills, and experience in home care is a plus. Qualifications & Skills
Education: Nursing Diploma or Associate’s degree in Nursing (RN license required)
Experience: 2-5 years minimum, with preferred 5-10 years for advanced roles Strong communication, coordination, and educational skills Ability to work collaboratively with healthcare teams and community stakeholders
Key Duties: Facilitate patient discharge planning and care coordination Maintain thorough documentation of all activities Participate in planning and marketing of home health services Educate staff and community about home care options and programs Comprehensive health, dental, vision, and life insurance Disability insurance and retirement plan with employer contributions Tuition assistance, flexible spending accounts, paid time off, and other resources
