About the role
Hartford HealthCare at Home seeks a Hospice Advanced Practice Registered Nurse (APRN) to deliver comprehensive, patient-centered hospice care. You will collaborate with an interdisciplinary team to manage symptoms, support goals of care, and guide families across care settings.
The role includes face-to-face eligibility encounters, recertifications, on-call coverage, and active participation in IDT meetings within a community-based care model. CT APRN license with prescriptive authority required.
