At Woodbury Heights Nursing and Rehabilitation center we are looking to build the best team to provide optimum care to our LTC and Rehabilitation residents. Woodbury Heights Nursing and Rehabilitation Center has an exciting opportunity for you. We take pride in our compassionate, patient-centered approach, and our team of dedicated LPNs is at the heart of everything we do. Schedules: MUST BE ALBE TO WORK A MINIMUM OF EVERY OTHER WEEKEND7am to 3pm, 3pm to 11pm or 11pm to 7amshifts on weekdays available as well. Responsibilities, but not limited to: Provide compassionate, patient-centered care to residents, helping them live comfortably and with dignity. Administer medications and treatments as prescribed by physicians. Monitor and document vital signs, symptoms, and changes in resident conditions. Assist in developing and implementing personalized care plans. Collaborate with RNs, physicians, and other healthcare professionals to ensure the highest standard of care. Respond to medical emergencies and provide immediate support. Maintain accurate and up-to-date medical records. Promote a positive, supportive environment for both residents and team members. Requirement: Valid LPN license in New York State Current CPR and First Aid certification 1 year of experience in long term care. Woodbury Heights is an Equal Opportunity Employer.
