Registered Nurse (RN): Per-Diem Longwood Hospice - North Shore AreaWe are seeking a compassionate and experienced Registered Nurse (RN)to join our hospice team. In this role, you will provide high-quality, patient-centered care to individuals and families during some of life’s most important moments. Key Responsibilities:
  • Provide direct nursing care through routine visits, admissions, and recertifications
  • Perform comprehensive patient and family assessments and develop individualized care plans
  • Conduct pain and symptom management assessments at every visit
  • Communicate changes in patient condition to the RN Case Manager and physician as needed
  • Maintain accurate, timely documentation in accordance with hospice policies
  • Participate in interdisciplinary team (IDT) meetings
  • Support patients and families during end-of-life care, including presence at time of death when possible
  • Supervise LPNs and Home Health Aides as needed
  • Ensure compliance with all regulatory, HIPAA, and infection control standards
  • Participate in ongoing training, quality improvement, and professional development
Qualifications:
  • CurrentRegistered Nurse (RN) licensein good standing
  • Minimum 2 years of nursing experience
  • Strong clinical, communication, and interpersonal skills
  • Ability to work independently with minimal supervision
  • Basic computer proficiency (MS Word, Excel, EMR systems)
  • Must pass background check and pre-employment requirements
Preferred Qualifications:
  • Previoushospice experience
  • Prior supervisory or leadership experience
  • Physical & Work Requirements
  • Ability to lift up to 50 lbs and perform physical tasks (walking, standing, bending, etc.)Ability to travel to patient homes on a daily basis
  • Comfortable working in varied home environments, including exposure to illness and other conditions
