About the role

Reserve Care (formerly Hospice of the Western Reserve) is seeking a Clinical Admissions Nurse to ensure admissions for patients in home, nursing facilities, or other care settings. You will collaborate with the Referral Department and physicians to promote hospice care and ensure timely certifications. The role requires nursing expertise, compassionate patient interaction, and adherence to HIPAA policies. Weekend schedule is Saturday and Sunday, 9:00 AM–9:30 PM, with 24 scheduled hours and
