- Evaluate, diagnose, and manage acute and chronic medical conditions in patients across the lifespan
- Conduct comprehensive patient assessments, including history and physical examinations
- Develop, implement, and monitor individualized treatment plans
- Order, interpret, and review laboratory tests, diagnostic imaging, and other diagnostic studies
- Prescribe medications and treatments in accordance with California regulations and organizational policies
- Provide preventive care services, including wellness exams, immunizations, health screenings, and chronic disease management
- Educate patients and families regarding diagnoses, treatment plans, medications, and healthy lifestyle choices
- Collaborate closely with physicians, specialists, nursing staff, and care coordinators to ensure continuity of care
- Document patient encounters accurately and timely within the EMR system
- Competitive salary range: $132,000–$198,000 annually

- Master’s degree or higher in Nursing from an accredited program
- Current California Nurse Practitioner license
- Board certification as a Nurse Practitioner (FNP preferred)DEA certification and furnishing license required
- Current BLS certification
- Previous experience in Family Medicine, Primary Care, Community Health, or Rural Health preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced clinical environment
- Experience managing chronic diseases such as diabetes, hypertension, and cardiovascular conditions
- Experience providing preventive and wellness care across all age groups
- Knowledge of evidence-based primary care practices
- Bilingual English/Spanish skills preferred
- Compassionate, patient-focused approach to care
