About the role

School Health & Wellness Social Worker (Grant Funded)The School Health & Wellness Social Worker supports SCBOCES member districts by providing behavioral health support, family engagement, crisis response, care coordination, and connections to community-based services. The social worker assists districts in strengthening MTSS systems, increasing access to supports, and improving student well-being and school connectedness. The School Health & Wellness Social Worker provides direct behavioral health and social-emotional support services to students and families. Coordinates referrals and access to community-based mental health, social service, and healthcare resources. Assists districts in implementing MTSS, PBIS, and behavioral health systems. Supports attendance improvement, dropout prevention, and family engagement initiatives. Participates in crisis intervention, safety planning, re-entry meetings, and crisis response activities. Collaborates with school staff, community agencies, and behavioral health providers. Assists with behavioral health screenings and referral processes. Provides consultation and resources to school staff and families. Supports prevention education and wellness initiatives. Assists with grant reporting, documentation, and data collection requirements. Participates in regional Wellness and Crisis Response Teams. Develops and evaluates a comprehensive approach to improve student mental health through early identification, prevention, and accessible interventions on site for SCBOCES districts. Provides input and feedback on the materials and interventions provided throughout the BOCES. Analyzes the impact of wellness programming on students' academic, social, emotional, and behavioral outcomes. Develops a plan to expand the mental health and wellness programming for school staff. Facilitates the development of resources such as a mental health toolkit for stakeholders. Coordinates and facilitates professional development opportunities with counselors, administrators, and other stakeholders as applicable and appropriate. Works alongside member school districts to provide quality therapy and consulting services. Attends meetings as required. Uses professional Code of Ethics and standards of practice to guide ethical decision making. Demonstrates the ability to communicate effectively, collaborate with co-workers, and interact with colleagues in a professional manner. Demonstrates a positive attitude towards change.
