Provider Must be Local - Only Mileage is Reimbursed

PENETRATING Trauma Cases
BLUNT Trauma Cases
Chest Tube Insertion
Laparoscopic Procedures Required:
Appendectomy
Cholecystectomy
Estimated Credentialing Timeframe: 30-40 days to start if all licensure is active and good standing
Submission Requirements
Board Certified - REQUIRED
Fellowship Training - REQUIRED
Active CT License - REQUIRED (can reconsider if no candidates available)
ACLS:
REQUIRED
Local Candidates - Preferred
