About the role
Overview:

Heartland Home Health is seeking a PRN/FLEX Home Health Social Worker (MSW) to provide emotional, social, and practical support for patients and their loved ones out of West Branch, MI. You’ll be part of an interdisciplinary team that helps individuals navigate serious illness, life transitions, and the complexity of home-based care. What You’ll Do as a Home Health Social Worker (MSW):Conduct psychosocial assessments to identify patient and family needs and develop care goals aligned with the individualized Plan of Care. Deliver emotional support, counseling, and crisis intervention services to patients and caregivers. Educate families on available community resources, advance directives, care planning, and coping strategies. Collaborate closely with nurses, therapists, physicians, and other members of the care team to ensure comprehensive, patient-centered care. Provide support and guidance through serious illness, loss, grief, and end-of-life decisions. Communicate changes in patient or family condition that may impact care planning, safety, or psychosocial well-being. Document all services and interventions thoroughly, accurately, and in accordance with regulatory and agency standards. Advocate for the dignity and rights of patients and families, while offering sensitivity, respect, and culturally competent care.
About You: Qualifications –
What You’ll Bring:
Master’s Degree in Social Work from a CSWE-accredited program Licensure as required by the state of practice At least 1 year of social work experience in a healthcare or home health/hospice setting Deep understanding of the psychosocial dynamics of illness, loss, and death, and the ability to provide support during times of high emotional need Knowledge of hospice and home health care services, including community resources, Medicare/Medicaid guidelines, and care coordination Strong communication, critical thinking, and documentation skills Emotional resilience and empathy—able to support others while maintaining personal well-being Preferred Experience (Not Required):Experience in home health or hospice social workEMR documentation (Home Care Home Base or similar system)Bereavement support, grief counseling, or crisis intervention Familiarity with interdisciplinary team collaboration in a clinical setting We Offer: Benefits for All Associates (Full-Time, Part-Time & Per Diem):Competitive Pay 401(k) with Company Match Career Advancement Opportunities National & Local Recognition Programs Teammate Assistance Fund Additional Full-
