- Coordinate and monitor person-centered services and supports
- Facilitate and oversee Comprehensive Functional Assessments (CFAs) within Intermediate Care Facilities (ICFs)
- Collaborate with interdisciplinary teams to develop, implement, and monitor individualized service plans
- Provide case management and service coordination for individuals receiving supports
- Ensure compliance with regulatory requirements, Medicaid standards, and agency policies
- Maintain accurate documentation, records, and reports
- Monitor program quality and advocate for the needs and goals of individuals served
- Provide leadership, guidance, and support to team members
- Communicate effectively with individuals served, families, staff, and community providers
- Identify service needs and implement proactive solutions to support positive outcomes
- Preferred Experience
- Experience working in residential, day habilitation, or community-based support programs
- Experience with person-centered planning and service coordination
- Background in Social Work, Human Services, Psychology, or related disciplines
