Job Description & Requirements Specialty: Home Health Discipline: RN Start Date: 10/26/2026 Duration: 16 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Registered Nurse Shift Information Monday - Friday 8:00 am - 4:30 pm Minimum Years of Experience 0.0 Job Description The Registered Nurse in Home Care plays a vital role in the Program of All-Inclusive Care for the Elderly (PACE) by providing both preventive and acute nursing care to participants. This position emphasizes compassionate, patient-centered care, allowing elderly individuals to live independently in their communities. Primary Purpose The Clinical Care Registered Nurse (CCRN) offers direct nursing care, applying advanced knowledge and skills to manage complex clinical situations. This role involves collaboration with both licensed and non-licensed personnel.
Duties and Responsibilities: Routine monitoring of acute and chronic medical conditions in alignment with participants' care goals. Perform wound care and treatments as outlined in the participant's care plan. Administer medications, immunizations, and other care as prescribed. Conduct assessments and actively participate in the development of care plans. Make home visits regularly to provide care and treatment as directed by Primary Care Providers (PCPs). Address on-call needs and assess participant requirements as necessary. Document patient encounters accurately in medical records per policy guidelines. Collaborate and communicate effectively with interdisciplinary team members and participants’ families. Stay informed of any issues and escalate them to the appropriate leadership. Complete additional duties as assigned.
Qualifications: Bachelor of Science in Nursing preferred; Associate degree required. Diploma or degree from an accredited School of Nursing or University. Proficient in Microsoft Word and Outlook. Strong written and verbal communication skills. Minimum of one year of experience with the frail elderly or long-term care population; home care experience preferred. Wound care experience preferred. Specific Requirements Current CPR certification by AHA or willingness to obtain it. Demonstrate core values of compassion, integrity, teamwork, and excellence. Ability to manage time and resources efficiently. Exceptional customer service skills supporting all staff and participants. Capability to make independent decisions as needed. Ability to plan, organize, develop, implement, and interpret organizational goals and policies. Maintain confidentiality regarding sensitive information. Current immunizations or medical documentation for waivers as applicable. Reliable transportation and a valid driver's license with vehicle insurance. Work Environment This role requires a visible presence in the community and entails frequent communication with staff, participants, and caregivers. The position may require extended hours and availability on weekends and holidays. The RN may be exposed to infectious waste and hazardous materials and will be required to operate a vehicle in varying weather conditions. Physical Demands Must be capable of coping with the mental and emotional challenges of the position. Responsibilities include prolonged sitting, standing, and lifting up to twenty pounds as necessary. Minimum Working Requirements Must be 18 years of age or older.
