ASSISTANT PROJECT MANAGER Department: GENESEE COMMUNITY HEALTH ACCESS PROGRAM (CHAP) Greater Flint Health Coalition – Flint, MI Greater Flint Health Coalition is seeking an Assistant Project Manager to join our team.
Overview:
Greater Flint Health Coalition (GFHC) is a nonprofit, collective impact organization. The GFHC functions as a neutral convener of numerous multi-sector coalitions as well as the operator of a portfolio of community-based programs focused on improving health status, increasing healthcare access and innovation, and addressing chronic disease, social determinants of health, health disparities, and a variety of emerging public health issues. A program of GFHC, Genesee CHAP (Community Health Access Program) is a community-based population health and care management program serving a diverse network of healthcare providers, physician organizations, health plans, and hospitals to collaboratively address the clinical and community needs impacting residents, patients, and beneficiaries, and their ability to both access health care and achieve optimal health outcomes. Genesee CHAP seeks to connect residents to a medical home, remove barriers to care typically associated with the social determinants of health, while raising the quality of care, better utilizing resources, and decreasing costs. Genesee CHAP's priority population is Flint and Genesee County children and adults covered by Medicaid insurance, as well as those residents who are uninsured and underinsured. The full-time Assistant Project Manager will support the Program Director with efforts related to the development, implementation, and management of Genesee CHAP and its community impact programs. The Assistant Project Manager will engage with providers, insurers, multi- sector coalition partners, and community members, support committee convening activities and functions, track and manage program performance data, and advance the mission of the GFHC.As a full-time Assistant Project Manager, this exciting position and your specific projects, tasks, and responsibilities will evolve over time. Daily responsibilities will include workplan management, data compilation and analysis, problem solving, development of communication materials, and program documentation. Flexibility and a commitment to continuous learning will be required of the successful candidate. A skill set based in public health, community engagement, and program implementation will be vital to this work to improve the factors that affect health via multi-sector partnership activities.
Primary Responsibilities:
Assist with the planning, support, and execution of committees and community collaboration projects in partnership with cross-functional teams composed of clinical partners, education partners, community and social service agencies, and the community at-large. Provide committee and task force staffing, ensuring responsiveness to partner requirements, inputs, and concerns. Collaborative processes include timely material preparation, agenda development, minutes, program/grant development, workplan execution, and the completion of required committee follow up resulting from consensus decision making. Manage project timelines, activities, documentation, and deliverables while maintaining alignment with multi-sector partner objectives and grant / project workplan requirements. Collect and analyze data to inform community needs and project outcomes; assist with planning, implementing, monitoring, and evaluating programs; prepare reports and share best practices with multi-sector stakeholders. Participate in quality assurance, continuous improvement efforts, identifying system and process issues. Support the mission, vision, values and key strategies of the Greater Flint Health Coalition and development of collaborative relationships with the organizations that interface with the program. Other duties and responsibilities as assigned. Qualifications & Experience Required: To be successful in this position, a candidate must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Bachelor's Degree in health or related field; Master's degree (MPH, MHSA, or MS) is a plus, but not required. Experience and a passion for public health programs, with an emphasis on those that address broad factors that affect the health of residents and seek to build alignment amongst multi-sector organizations in a collaborative, team-oriented style.
