Responsibilities:
Develop and implement individualized treatment plans for adult and geriatric patients, focusing on functional improvement, mobility, and pain reduction.
Deliver manual therapy interventions, including trigger point techniques, soft‑tissue mobilization, and joint mobilization as clinically appropriate.
Monitor patient progress, ensure safety during all therapeutic activities, and provide ongoing motivation throughout the rehabilitation process.
Educate patients and families on therapeutic exercises, home‑based recovery strategies, and long‑term wellness practices.
Document evaluations, treatment sessions, and progress notes accurately within eClinicalWorks in compliance with regulatory and clinical standards.
Collaborate with interdisciplinary teams to support coordinated, patient‑centered care and optimize clinical outcomes.
Qualifications:
Licensed Physical Therapist 1+ years of experience as a Physical Therapist in an outpatient, inpatient, or rehabilitation setting.
Fluency in Spanish preferred to support patient communication and education.
Strong communication skills with the ability to collaborate effectively within multidisciplinary teams and build rapport with diverse patient populations.
