About the role
- Lead daily clinical operations, including staffing, scheduling, and care coordination
- Supervise and mentor nurses, home health aides, and interdisciplinary team members
- Ensure all patient care aligns with physician orders, care plans, and regulatory standards
- Review and audit clinical documentation for accuracy, compliance, and quality
- Partner with physicians and care teams to deliver seamless, coordinated care
- Support onboarding, training, and ongoing staff development
- Drive performance improvement initiatives and participate in QAPI programs
- Provide clinical support in the field as needed, including complex patient cases
- Step in as a leadership resource in the absence of the Director of Patient Care Services
- What You BringActive California RN license 2+ years of recent home health or related clinical experience
- Proven leadership or supervisory experience preferred
- Strong clinical judgment and knowledge of home health regulations (Title 22, CMS, etc.)Excellent communication, organization, and team leadership skills
