About the role

ESSENTIAL FUNCTIONS & RESPONSIBILITIES:Serve as a care manager, navigator, and advocate for members of the LiveWell program. Serve as a champion of healthy aging practices for all program members to improve independence and support members in their goals of living at home as they age. Conduct regular health and wellness screenings to help members set and work toward goals for improvement in health and functional ability. Coordinate, plan, and oversee member care and services when support is needed at home. Collect, track, and analyze member health and wellness data to help members meet their goals and improve levels of independence. Coordinate wellness activities, community resources, vendors, and interventions that meet the interests and needs of the membership. Collaborate across all Blakeford Senior Life service lines and support program growth and outreach.
