About the role

WelbeHealth in Carson, CA is seeking a Home Care Coordinator Supervisor reporting to the Home Care Manager. You will oversee a team of Home Care Coordinators, mentor staff, and coordinate with MOE and IDT meetings to ensure high-quality care. This on-site role requires healthcare licensure or equivalent, at least three years in home-based case management or nursing for frail seniors, and CPR/First Aid certification within 180 days.
