About the role

Alignment Health is seeking an Advanced Practice Clinician to join the Care Anywhere (CAW) program to provide in-home assessments, care management, and patient education. You will collaborate with Regional Medical Officer and care teams to develop tailored care plans for seniors and frail patients, ensuring timely interventions and high-quality outcomes. Must hold NP/PA license with Master’s degree and be willing to travel to patients’ homes.
