Responsibilities: Provide comprehensive primary care to patients across the lifespan Conduct physicals, wellness visits, acute visits and follow-up care Evaluate, diagnose and manage acute and chronic conditions Provide preventive health counseling and patient education Coordinate care with specialists and other healthcare team members Utilize telemedicine when appropriate Maintain accurate and timely electronic medical records Current New York State Nurse Practitioner or Physician Assistant license Graduate of an accredited NP or PA program Family medicine or primary care experience preferred Strong clinical, communication and interpersonal skills Commitment to patient-centered, compassionate care New graduates with a strong interest in family medicine are encouraged to apply
