Benefits: Sign-On / Retention Bonus: Up to $15,000POSITION OVERVIEWTelos Health Systems is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to serve as an in-house SNF-ist, providing comprehensive general medicine care to geriatric residents in a skilled nursing facility. As an NP/PA with Telos, you will provide longitudinal care to short-term rehabilitation and long-term care residents while working closely with facility care teams to improve patient outcomes and reduce unnecessary hospitalizations. Responsibilities include: • Manage the medical care of short-term rehabilitation and long-term care residents• Perform comprehensive health assessments and physical examinations• Diagnose and treat acute and chronic medical conditions• Prescribe medications and treatments within your scope of practice• Evaluate residents experiencing changes in condition• Conduct routine and monthly stability visits• Participate in advanced care planning and goals-of-care discussions• Collaborate with nursing staff, medical directors, attending physicians, specialists, and interdisciplinary care teams• Facilitate referrals to specialists and other healthcare providers as appropriate• Maintain accurate clinical documentation and CPT coding• Follow established clinical protocols and best practices• Help identify opportunities to prevent avoidable emergency department visits and hospitalizationsWHAT YOU'LL NEED
Required: • Active, unencumbered Nurse Practitioner (APRN/APN) or Physician Assistant (PA-C) license in North Carolina• Current DEA registration• Master's degree from an accredited Nurse Practitioner or Physician Assistant program• Strong clinical judgment and communication skills• Ability to work independently while collaborating effectively with facility-based care teams• Strong organizational and documentation skills
Preferred: • At least 1 year of experience in a Skilled Nursing Facility (SNF), long-term care, post-acute care, or similar setting• Experience caring for geriatric or medically complex populations• Experience managing acute and chronic conditions in a post-acute settingFULL-TIME BENEFITSAvailable to full-time employees working at least 30 hours per week.
