About the role
- Providing skilled nursing visits according to physician orders and individualized plans of care
- Assessing patients and monitoring changes in their condition
- Administering prescribed medications, treatments, and nursing interventions
- Educating patients and caregivers regarding medications, diagnoses, treatments, and ongoing care needs
- Completing accurate and timely clinical documentation
- Communicating changes in patient status to the appropriate members of the care team
- Collaborating with physicians, therapists, case managers, and other healthcare professionals
- Supporting patient safety, comfort, independence, and overall quality of life
- Participating in the required on-call rotation and responding to patient needs as appropriate
- We are looking for an LPN who can combine strong clinical skills with excellent communication and time management.
- Graduate of an accredited Practical Nursing or Vocational Nursing program
- Active and unrestricted Minnesota LPN license
- Current CPR certification
- One or more years of clinical nursing experience preferred
- Previous home health experience is preferred but not required
- Valid driver's license and dependable transportation
- Ability to travel between patient residences within the assigned service area
- Strong clinical documentation, communication, and organizational skills
- Ability to independently organize patient visits and manage changing priorities
