What You'll DoConduct psychosocial assessments and develop individualized care plans Support residents and families with discharge planning, adjustment to facility life, and advocacy Coordinate with nursing, activities, and administration to ensure residents' emotional and social needs are met Maintain accurate, timely documentation in compliance with state and federal regulations Serve as a liaison between residents, families, and community resources What We're Looking ForMaster's degree in Social Work (MSW) Active state licensure (LSW/LCSW or equivalent) in good standing Prior experience in long-term care, skilled nursing, or a healthcare setting preferred Strong communication skills and a genuine passion for resident-centered care Ability to pass required background check, TB screening, and other pre-employment clearances
