Spiritual Care Coordinator Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them. Community Healthcare of Texas is currently recruiting for a Spiritual Care Coordinator.
POSITION SUMMARY:
The Spiritual Care Coordinator provides spiritual and emotional support for patients, families, and staff; assists in connecting families with avenues of spiritual support and provides ongoing bereavement care.
RESPONSIBILITIES: Assess patient/family with spiritual care needs; participate in the plan of care
Provide spiritual and emotional support according to the plan of care Participate in on-call rotation and various schedules/duties as assigned or needed Document visits/phone calls according to Policy Participate in Interdisciplinary Team meetings Provide aftercare and bereavement support in cooperation with the Bereavement Coordinator Provide spiritual and emotional support for staffPOSITION QUALIFICATIONSBachelor's degree in Religious Studies required.
