About the role

Alternate Solutions Health Network seeks a Community Care Coordinator to help patients transition to home-based care. You will coordinate services, review documentation, and advocate for patients across care settings. As part of the Columbus team, you’ll build relationships with facilities, physicians, and referral sources, ensuring safe, effective discharge planning and continuity of care for patients in our post-acute network.
